SnK Army app form

This is the application form for group.

Save the PNG image or download the PSD to fill it in.
The application MUST contain all the information required on the left and a full body drawing of your OC on the right.

You can add head shots or other details of your character in the free space but it is not mandatory. In the description you may add other information about your character's background if you want. (But remember, for now we do not allow titan shifters. If we allowed that, everyone would be one, right?)
